Please complete the form below to request a DBS Application Form for your DBS Check - BWSW will use this form to assess your eligiblity for a check so please complete the questions fully and accurately.

Please note: You must be a current member of British Water Ski & Wakeboard in order for us to process a DBS check

DBS Eligibility Information

The following questions will determine your eligibility for a DBS Check within the sport of water skiing and wakeboarding.
